Acidity, generated in hypoxia or hypermetabolic states, perturbs homeostasis and is a feature of solid tumors. That acid peripherally disperses lysosomes is a three‐decade‐old observation, yet one little understood or appreciated. However, recent work has recognized the inhibitory impact this spatial redistribution has on mechanistic target of rapamycin complex 1 (mTORC1), a key regulator of metabolism. This finding argues for a paradigm shift in localization of mTORC1 activator Ras homolog enriched in brain (RHEB), a conclusion several others have now independently (...) reached. Thus, mTORC1, known to sense amino acids, mitogens, and energy to restrict biosynthesis to times of adequate resources, also senses pH and, via dampened mTOR‐governed synthesis of clock proteins, regulates the circadian clock to achieve concerted responses to metabolic stress. While this may allow cancer to endure metabolic deprivation, immune cell mTOR signaling likewise exhibits pH sensitivity, suggesting that suppression of antitumor immune function by solid tumor acidity may additionally fuel cancers, an obstacle potentially reversible through therapeutic pH manipulation. Objectives: The official implementation of clinical pharmacy in Vietnam has arrived relatively late, resulting in various stressors. This study aims to evaluate job stress level and suggest viable solutions.Methods: A cross-sectional study was conducted on clinical pharmacists in 128 hospitals in Ho Chi Minh City. Job stress questions were derived from the Healthcare Profession Stress Inventory.Results: A total of 197 CPs participated, giving a response rate of 82.4%. Participants were found to have moderate job stress with an overall mean stress (...) score of 1.5 and stress rate of 52.8%. The sample size was statistically adequate and the HPSI was valid and reliable. Patient care responsibility was the main stressor, especially in public hospitals, followed by job conflicts. Lack of experience, low income, and inability to participate in clinical ward rounds caused significant stress to CPs regarding job recognition and job uncertainty. More practice-oriented training programs in bachelor curricula and clinical practice should be applied to help CPs gain more experience, self-confidence, and diminish job stress.Conclusion: CPs in HCMC have moderate stress. More practice-oriented training programs should be prioritized to lessen stress for CPs. (shrink)

BackgroundThe informed consent process in clinical trials has been extensively studied to inform the development processes which protect research participants and encourage their autonomy. However, ensuring a meaningful informed consent process is still of great concern in many research settings due to its complexity in practice and interwined socio-cultural factors.ObjectivesThis study explored the practices and meaning of the informed consent process in two clinial trials conducted by Oxford University Clinical Research Unit in collaboration with the Hospital for Tropical Diseases in (...) Ho Chi Minh City, Vietnam.MethodsWe used multiple data collection methods including direct observervations, in-depth interviews with study physicians and trial participants, review of informed consent documents from 2009 to 2018, and participant observation with patients’ family members. We recruited seven physicians and twenty-five trial participants into the study, of whom five physicians and thirteen trial participants completed in-depth interviews, and we held twenty-two direct observation sessions.ResultsWe use the concept “fragmented understanding” to describe the nuances of understanding about the consent process and unpack underlying reasons for differing understandings.ConclusionsOur findings show how practices of informed consent and different understanding of the trial information are shaped by trial participants’ characteristics and the socio-cultural context in which the trials take place. Abstractc‐Rel is a proto‐oncogene first identified as the cellular counterpart of the v‐Rel oncogene derived from the avian reticuloendotheliosis retrovirus (REV‐T). It was subsequently discovered that c‐Rel belongs to the NF‐κB/Rel transcription factor family whose members share a common DNA recognition motif and similar signaling pathways. Despite the similarities, however, each NF‐κB/Rel member possesses unique properties with regard to tissue expression pattern, response to receptor signals and target gene specificity. These differences are fairly evident from the non‐redundant phenotypes exhibited by (...) individual NF‐κB/Rel knockout mice. Hence the work described in this review will compare and contrast the various physiological functions of c‐Rel to those of other NF‐κB members, particularly with respect to the regulation of proliferation, survival and effector functions in multiple hematopoietic and immunological cell types. The study of c‐Rel knockout mice in several disease models will also be discussed as they reveal an important role for c‐Rel in response to allergens, auto‐antigens, allo‐antigens and pathogenic infection. The robustness of the classical tests on means (Z, t, and F) and variances (chi square and F) was investigated by obtaining 30,000 (or, sometimes, 10,000 or 150,000) values of the test statistic under assumption-violating conditions and comparing the actual proportion of Type I errors with the proportion expected when all assumptions are met. The sampling and testing conditions investigated were: population shape (L-shape or bell-shape), relative population variance (1 or 4), sample size (8, 16, or 24), nominal significance level (...) (.05,.01, or.001), and location of rejection region (left-tailed, right-tailed, or two-tailed). All tests on means were nonrobust under most of the investigated conditions, and one was nonrobust under all of them. Both tests on variances were extremely nonrobust under virtually all conditions. The worst nonrobustness of the one- and two-independent-sample Z and t tests rivaled or exceeded that of the notoriously nonrobust chi-square and F tests on variances. (shrink)

Defect detection is a major concern in quality control of various products in industries. This paper presents two different machine-vision based methods for detecting defects on periodically patterned textures. In the first method, input defective image is split into several blocks of size same as the size of the periodic unit of the image and chi-square histogram distances of each periodic block with respect to itself and all other periodic blocks are calculated to get a dissimilarity matrix. This dissimilarity matrix (...) is subjected to Ward's hierarchical clustering to automatically identify defective and defect-free blocks. The second method of defect detection is based on Universal Quality Index which is a measure of loss of correlation, luminance distortion and contrast distortion between any two signals. Quality indices of a periodic block with respect to itself and all other periodic blocks are calculated to get a similarity matrix containing quality indices. Specific variances of the periodic blocks are derived from the quality index matrix through orthogonal factor model based on eigen decomposition. These variances are subjected to Ward's hierarchical clustering to automatically identify defective and defect-free blocks. Results of experiments on real fabric images with defects show that the defect detection methods based on chi-square histogram distance and universal quality index yield a success rate of 98.6% and 97.8% respectively. Estos escritores dejan traslucir en sus obras sus inquietudes respecto a una influencia que (...) simultáneamente buscan y temen sobre los lectores. Sus «libros dentro de los libros» pueden ser interpretados como una vacuna con la que el escritor quiere proveer al lector de las herramientas para protegerse de los efectos perversos de una literatura recibida en clave romántica. Proponemos, siguiendo especialmente a Renato Poggioli, John Freccero o René Girard, una interpretación antirromántica que subraya la actualidad de la arquitectura moral de la Divina Comedia y su ascendencia sobre varios clásicos de la literatura moderna posterior. BackgroundHealthcare ethics is neglected in clinical practice in LMICs such as Nepal. The main objective of this study was to assess the current status of knowledge, attitude and practice of healthcare ethics among resident doctors and ward nurses in a tertiary teaching hospital in Nepal.MethodsThis was a cross sectional study conducted among resident doctors and ward nurses in the largest tertiary care teaching hospital of Nepal during January- February 2016 with a self-administered questionnaire. A Cramer’s V value was assessed to (...) ascertain the strength of the differences in the variables between doctors and nurses. Association of variables were determined by Chi square and statistical significance was considered if p value was less than 0.05.ResultsOur study demonstrated that a significant proportion of the doctors and nurses were unaware of major documents of healthcare ethics: Hippocratic Oath, Nuremberg code and Helsinki Declaration. A high percentage of respondents said that their major source of information on healthcare ethics were lectures, books, and journals. Attitude of doctors and nurses were significantly different in 9 out of 22 questions pertaining to different aspects of healthcare ethics. More nurses had agreement than doctors on the tested statements pertaining to different aspects of healthcare ethics except for need of integration of medical ethics in ungraduate curricula,paternalistic attitude of doctor was disagreed more by doctors. Notably, only few doctors stood in support of physician-assisted dying.ConclusionsSignificant proportion of doctors and nurses were unaware of three major documents on healthcare ethics which are the core principles in clinical practice. Provided that a high percentage of respondents had motivation for learning medical ethics and asked for inclusion of medical ethics in the curriculum, it is imperative to avail information on medical ethics through subscription of journals and books on ethics in medical libraries in addition to lectures and training at workplace on medical ethics which can significantly improve the current paucity of knowledge on medical ethics. Although CSR scholarship has highlighted how tensions in CSR implementation are negotiated, little is known about its normative and moral dimension at a micro-level. Drawing upon the economies of worth framework, we explore how spirituality influences the negotiation of CSR tensions at an individual level, and what types of justification work they engage in when experiencing tensions. Our analysis of semi-structured interview data from individuals who described themselves as Buddhist and were in charge of CSR implementations for their organizations shows (...) that spirituality influences how they compromise among competing moral values by identifying two forms of justification work: compartmentalizing work and contextualizing work, which help spiritual practitioners minimize moral dissonance. About thirty years ago, when the Internet started to be commercialised, access to the medium became a topic of research and debate. Up-to-date evidence about key predictors, such as age, is crucial because of the Internet's ever-changing nature and the challenges associated with gaining access to it. This paper aims to give an overview of New Zealand's Internet access trends and how they relate to age. It is based on secondary analysis of data from a larger online panel survey with (...) 1,001 adult respondents. The Chi-square test of independence and Cramer's V were used in the analysis. The study provides new evidence to understand the digital divide. Specifically, it uncovers a growing disparity in the quality of Internet connectivity. Even though fibre is the most common type of broadband connection at home, older adults are less likely to have it and more likely to use wireless broadband, which is a slower connection type. Additionally, a sizable majority of people in all age categories have favourable opinions on the Internet. Interestingly, this was more prevalent among older people, although they report an increased concern about the security of their personal information online. The implications of the results are discussed and some directions for future research are proposed. (shrink)

BackgroundDouble-blind peer review has been proposed as a possible solution to avoid implicit referee bias in academic publishing. The aims of this study are to analyse the demographics of corresponding authors choosing double-blind peer review and to identify differences in the editorial outcome of manuscripts depending on their review model.MethodsData includes 128,454 manuscripts received between March 2015 and February 2017 by 25 Nature-branded journals. We investigated the uptake of double-blind review in relation to journal tier, as well as gender, country, (...) and institutional prestige of the corresponding author. We then studied the manuscripts’ editorial outcome in relation to review model and author’s characteristics. The gender (male, female, or NA) of the corresponding authors was determined from their first name using a third-party service (Gender API). The prestige of the corresponding author’s institutions was measured from the data of the Global Research Identifier Database (GRID) by dividing institutions in three prestige groups with reference to the 2016 Times Higher Education (THE) ranking. We employed descriptive statistics for data exploration, and we tested our hypotheses using Pearson’s chi-square and binomial tests. We also performed logistic regression modelling with author update, out-to-review, and acceptance as response, and journal tier, author gender, author country, and institution as predictors.ResultsAuthor uptake for double-blind submissions was 12% (12,631 out of 106,373). We found a small but significant association between journal tier and review type (p value < 0.001, Cramer’s V = 0.054, df = 2). We had gender information for 50,533 corresponding authors and found no statistically significant difference in the distribution of peer review model between males and females (p value = 0.6179). We had 58,920 records with normalised institutions and a THE rank, and we found that corresponding authors from the less prestigious institutions are more likely to choose double-blind review (p value < 0.001, df = 2, Cramer’s V = 0.106). In the ten countries with the highest number of submissions, we found a large significant association between country and review type (p value < 0.001, df = 10, Cramer’s V = 0.189). The outcome both at first decision and post review is significantly more negative (i.e. a higher likelihood for rejection) for double-blind than single-blind papers (p value < 0.001, df = 1, Cramer’s V = 0.112 for first decision; p value < 0.001; df = 1, Cramer’s V = 0.082 for post-review decision).ConclusionsThe proportion of authors that choose double-blind review is higher when they submit to more prestigious journals, they are affiliated with less prestigious institutions, or they are from specific countries; the double-blind option is also linked to less successful editorial outcomes. Various authors have recently expressed doubts about the public relevance of philosophy. These doubts target both academic philosophy in general and particular subfields of philosophy. This paper investigates whether these doubts are justified through two tests in which the lack of public relevance of a philosophical paper is operationalized as the degree to which that paper is isolated. Both tests suggest that academic philosophy in general is more isolated from the broader public than it should be, and confirm the hypothesis (...) that some subfields of philosophy are more isolated than others. We argue that this lack of public relevance is caused by the incentive structure of academic philosophy and discuss a range of individual-level and incentive-level solutions. (shrink)

It has recently been argued that the concept of natural kinds should be eliminated because it does not play a productive theoretical role and even harms philosophical research on scientific classification. We argue that this justification for eliminativism fails because the notion of ‘natural kinds’ plays another epistemic role in philosophical research, namely, it enables fruitful investigation into non-arbitrary classification. It does this in two ways: first, by providing a fruitful investigative entry into scientific classification; and second—as is supported by (...) bibliometric evidence—by tying together a research community devoted to non-arbitrary classification. The question of eliminativism then requires weighing off the benefits of retaining the concept against its harms. We argue that the progressive state of philosophical work on natural kinds tips this balance in favour of retaining the concept. (shrink)

Come pensavano il tempo i Greci di eta arcaica, prima che Platone ed Aristotele ne formulassero una definizione? Nei miti, il tempo della natura, ciclico e rassicurante, non era messo a confronto con quello umano, imprevedibile anche se inesorabilmente determinato. Il ritmo scandito dal sole non era sovrapponibile a quelle della vita umana, l'avvicendarsi delle stagioni non misurava la durata della stirpe. Solo in eta sapienziale (VII-V secolo a.C.) si comincio a pensare il tempo in modo onnicomprensivo e si avverti (...) l'esigenza di misurarne il flusso. In quell'epoca, Eraclito tratteggio una concezione del tempo assolutamente innovativa, capace di ribaltare le visione del mondo trasmessa dai miti. Penso al tempo delle stagioni in analogia con quello interiore e vide nell'inesauribile creativita dell'anima umana lo specchio dell'infinita potenzialita della natura. La concezione offerta da Eraclito, avversata dai filosofi di eta classica e misconosciuta dalla scienza, trasmette ancora oggi, a chi legge con attenzione i frammenti rimasti della sua opera, un fiducioso messaggio di speranza nella capacita umana di vivere creativamente il tempo.
